Privacy Practice

The Administrative Simplification provisions of the Health Insurance Portability and Accountability Act of 1996, commonly known as HIPAA, include new rules that seek to improve efficiency and effectiveness of the healthcare system. A portion of HIPAA, referred to as the Privacy Rule, establishes a national standard for the collection, maintenance, access, use and disclosure of individually identifiable health information. Individually identifiable health information is considered to be protected health information. In addition, there are several individual rights that are defined in the Privacy Rule.

This Notice of Privacy Practices reflects our commitment to protecting your individually identifiable health information.

If you have questions or comments related to the HIPAA Privacy Rule, the Notice of Privacy Practices includes the Davis Vision Privacy Office contact information.
